Το μέγεθος του προβλήματος των ρευματικών παθήσεων στην Ελλάδα, Ελληνικό 'Ιδρυμα Ρευματολογικών Ερευνών

Το μέγεθος του προβλήματος των ρευματικών παθήσεων καθορίζεται από δύο παράγοντες:
  • Από τον επιπολασμό τους, δηλ. από τη συχνότητά τους, στο γενικό πληθυσμό
  • Από τις επιπτώσεις  τους στους ίδιους τους ασθενείς και στις οικογένειές τους, στο κοινωνικό σύνολο, στο σύστημα υγείας και στην εθνική οικονομία.
Πρόσφατα το Ελληνικό Ίδρυμα Ρευματολογικών Ερευνών, προκειμένου να μελετήσει το μέγεθος του προβλήματος των ρευματικών παθήσεων στην Ελλάδα, αλλά και πολλές άλλες παραμέτρους που σχετίζονται με τις παθήσεις αυτές,  οργάνωσε και πραγματοποίησε με τη συνεργασία πολλών γιατρών ρευματολόγων την πρώτη πανελλήνια επιδημιολογική έρευνα για τις ρευματικές παθήσεις (Ελληνικά / Αγγλικά) στο γενικό πληθυσμό ενηλίκων (άτομα ηλικίας 19 ετών και πάνω) της χώρας μας. Πολλά από τα ευρήματα της έρευνας έχουν ανακοινωθεί σε πολλά πανελλήνια και διεθνή ρευματολογικά συνέδρια και έχουν δημοσιευθεί σε ελληνικά και διεθνή ιατρικά περιοδικά.
Η έρευνα διεξήχθη σε 2 αστικές, 2 ημιαστικές και 5 αγροτικές περιοχές στη Βόρεια, Κεντρική και Νότια Ελλάδα με συνολικό πληθυσμό ενηλίκων 14.233 (Εικόνα 1).

Οι γιατροί ρευματολόγοι, που πραγματοποίησαν την έρευνα, επισκέφθηκαν τους ενήλικες κατοίκους των περιοχών αυτών στα σπίτια τους είτε πόρτα-πόρτα είτε μετά από τηλεφωνική προσυνεννόηση. Το ποσοστό συμμετοχής στην έρευνα ήταν πολύ μεγάλο, 82%.
Παρακάτω αναφέρονται μερικά από τα σημαντικότερα ευρήματα της έρευνας σε σχέση με τη συχνότητα και τις επιπτώσεις των ρευματικών παθήσεων στο γενικό πληθυσμό των ενηλίκων της χώρας μας.

Ποια είναι η συχνότητα των ρευματικών παθήσεων στο γενικό πληθυσμό;
 

  • Οι ρευματικές παθήσεις είναι πολύ συχνές, αφού ο συνολικός επιπολασμός τους, δηλ. η συνολική συχνότητά τους,  βρέθηκε στο επίπεδο του 26,9% των ενηλίκων. Με άλλα λόγια 1 στους 4 Έλληνες ενήλικες παρουσιάζει κάποια ρευματική πάθηση. Συνολικά 2.345.000 ενήλικες Έλληνες πάσχουν από ρευματικές παθήσεις.
  • Οι ρευματικές παθήσεις προσβάλλουν σημαντικά συχνότερα τις γυναίκες από ό,τι τους άνδρες, αφού 33,7% των ενήλικων γυναικών (συνολικά 1.497.000 γυναίκες) και 19,9% των ενήλικων ανδρών (συνολικά 848.000 άνδρες) προσβάλλονται από ρευματική πάθηση.
  • Η συχνότητα των ρευματικών παθήσεων αυξάνει με την πρόοδο της ηλικίας.

Ποιες είναι οι επιπτώσεις των ρευματικών παθήσεων στους ίδιους τους ασθενείς, στις οικογένειές τους, στο κοινωνικό σύνολο, στο σύστημα υγείας και στην εθνική οικονομία;

Σε σύγκριση με όλες τις άλλες ομάδες παθήσεων στο επίπεδο του γενικού πληθυσμού ενηλίκων:
  • Οι ρευματικές παθήσεις είναι το πιο συχνό χρόνιο πρόβλημα υγείας, αντιπροσωπεύοντας το 39% όλων των χρόνιων παθήσεων. Ακολουθούν, ως δεύτερο αίτιο, οι καρδιαγγειακές παθήσεις με ποσοστό ευθύνης 25%.
  • Οι ρευματικές παθήσεις είναι το πρώτο αίτιο και ευθύνονται για το 47% της  μακροχρόνιας λειτουργικής ανικανότητας. Ακολουθούν, ως δεύτερο αίτιο, οι καρδιαγγειακές παθήσεις με ποσοστό ευθύνης 22%.
  • Οι ρευματικές παθήσεις είναι το πρώτο αίτιο και ευθύνονται για το 26% του βραχυχρόνιου περιορισμού των  δραστηριοτήτων, όπως απουσίες από την εργασία κ.λπ. Ακολουθούν, ως δεύτερο αίτιο, οι παθήσεις του αναπνευστικού συστήματος με ποσοστό ευθύνης 22%.
  • Οι ρευματικές παθήσεις είναι το πρώτο αίτιο και ευθύνονται για το 20% όλων των ιατρικών επισκέψεων. Ακολουθούν, ως δεύτερο αίτιο, οι καρδιαγγειακές παθήσεις με ποσοστό ευθύνης 18%.
  • Οι ρευματικές παθήσεις είναι μετά τις καρδιαγγειακές παθήσεις το δεύτερο αίτιο και ευθύνονται για το 24% της λήψης φαρμάκων με ιατρική συνταγή.
  • Οι ρευματικές παθήσεις είναι μετά τις παθήσεις του αναπνευστικού συστήματος το δεύτερο αίτιο και ευθύνονται για το 18% της λήψης φαρμάκων χωρίς ιατρική συνταγή.     
  • Οι ρευματικές παθήσεις είναι το κυριότερο αίτιο νοσηρότητας, απουσιών από την εργασία και μακροχρόνιας λειτουργικής ανικανότητας στον εργαζόμενο πληθυσμό, δηλ. σε άτομα ηλικίας 19-65 ετών.

Συμπεράσματα

Είναι, επομένως, σαφές ότι οι ρευματικές παθήσεις αποτελούν ένα μέγιστο και πολυδιάστατο πρόβλημα δημόσιας υγείας, Γι’ αυτό θα πρέπει να λαμβάνονται σοβαρά υπόψη από τους αρμόδιους φορείς της Πολιτείας κατά τη λήψη αποφάσεων στα πλαίσια του σχεδιασμού και προγραμματισμού:
  • Της προπτυχιακής εκπαίδευσης, αλλά και της μεταπτυχιακής και της   συνεχιζόμενης δια βίου ιατρικής εκπαίδευσης σε θέματα Ρευματολογίας των γιατρών άλλων ειδικοτήτων, όπως π.χ. των γιατρών που ειδικεύονται ή έχουν ειδικευτεί στην Παθολογία και στη Γενική Ιατρική.
  • Της έρευνας.
  • Της παροχής των υπηρεσιών υγείας.
Σήμερα, χάρις στις σημαντικές προόδους της Ρευματολογίας , του κλάδου δηλ. της Ιατρικής που ασχολείται με τη μελέτη, την έρευνα, τη διάγνωση, την πρόληψη και τη θεραπεία των ρευματικών παθήσεων, είναι εφικτή η αποτελεσματική θεραπευτική αντιμετώπιση των παθήσεων αυτών, επιτυγχάνοντας καταστολή και ύφεση των κλινικών τους εκδηλώσεων, με δύο προϋποθέσεις:
  • Την έγκαιρη διάγνωση
  • Την πρώιμη και ορθή θεραπευτική παρέμβαση
Οι προϋποθέσεις αυτές μπορεί να εξασφαλιστούν με τη λήψη από τους αρμόδιους φορείς της Πολιτείας των παραπάνω μέτρων και την εφαρμογή τους σε πανελλήνια κλίμακα.

Less than 5% of individuals who are HLA B27 positive develop ankylosing spondylitis. However, in relatives of AS patients, who are HLA B27 positive, 20% of them will develop AS.

Although HLA B27 is the primary susceptibility gene in AS, it is not a pre-requisite for developing AS.  Homozygous HLA B27 tends to have a more severe disease as opposed to heterozygous HLA B27. The sensitivity of HLA B27 is also variable depending on the population in question. So local data may be important. Non-HLA genes may also have a role. There are other gene loci that may also be associated with AS, eg ERAP 1. Apart from ERAP1, IL23R polymorphisms or IL17 polymorphisms may have a role.

Here we are introduced to the existence of HLA B27 negative AS, where the diagnosis is delayed by 11.5 years as opposed to 8.5 years. In those who are HLA B27 negative, screening other HLA regions HLA B60 may yield positive results. There are 70 HLA B27 subtypes with HLA B2*705 being the most common. HLA B2*706 and HLA B2*709 is not associated with AS however.

HLA B27 misfolding has been implicated in disease pathogenesis.  HLA-B27 misfolding is associated with Endoplasmic Reticulum stress that results in activation of the Unfolded Protein Response(UPR).

Although the human spine evolved over millions of years to support a creature that walked upright (we were likely back pain free before that!), our spine still carries with it traces of our ancestry. For most primates, the spine serves as a “suspension bridge” from which all other structures and vital organs hang down. The primate spine is quite efficient when used this way, but the human spine went from being a horizontal suspension bridge to a vertical support column – a very dramatic shift. While our spine is definitely better suited for vertical support than, say, a chimp’s spine, our spine is more like a random compromise between suspension bridge and column than a completely different structure from that of our primate cousins. Back pain (also known as dorsalgia) is pain felt in the back that usually originates from the muscles, nerves, bones, joints or other structures in the spine.
Back pain can often be divided into neck pain, upper back pain, lower back pain or tailbone pain. Back pain may have a sudden onset or can be a chronic pain; back pain can be constant or intermittent, stay in one place or radiate to other areas. The spine is a complex interconnecting network of nerves, joints, muscles, tendons and ligaments, and all are capable of producing back pain. Large nerves that originate in the spine and go to the legs and arms can make pain back radiate to the extremities.

Stretching should be an integral part of each and every day.  Stretching is good not only for the arthritis pain sufferer; but for everyone.  Stretching helps to warm up muscles and tendons.  This makes them more pliable and loose which will result in reduced likelihood to injury and wear and tear. Try to spend 10 minutes each day stretching every major muscle group.
Once you have stretched or exercised, cool down properly.  Rest in a cool environment preferably with a cool spray mist of water.  Apply cold compresses to areas that may create arthritis pain and swelling.
Protect your joints. Don’t keep your joints in the same position as they tend to stiffen with sedentary exposure.  Always be aware of what you are getting ready to do and use  the strongest available joints available for whatever the task.
Exercise every single day of your life.. Exercise can help reduce arthritis pain and joint stiffness.  It increases flexibility and muscle strength. It can also help with weight control (see next paragraph), stress management, and helps you feel better in general. The Arthritis Foundation can provide a great list of exercises to help reduce arthritis pain and swelling.
Always try to keep proper balance of your weight.  Being overweight can create strain and impact on muscles and weight bearing joints all the time, not just with exercise.  Arthritis pain will proliferate.  Controlling your weight (or losing weight) can actually reduce the risk of osteoarthritis, can slow the onset of regular arthritis and even reduce the pain of those currently with arthritis.
Consider getting a massage. Massage therapy can relieve your pain, soothe stiff sore muscles, reduce inflammation and swelling. Make sure you utilize someone with experience in massage therapy; or, consider taking massage classes to learn for yourself or loved one.
Get a confirmed diagnosis of your arthritis or whatever your pain and/or swelling may be. If you are experiencing symptoms like pain, stiffness and swelling, you should consider seeing a doctor and getting a proper diagnosis. Remember that there are over one hundred different types of arthritis. It is important to get the specific diagnosis for the type of arthritis you have so that you can treat it.
Take your medication according to your doctor’s instructions.  Some treatments may take longer than others.  Don’t stop taking your medication just because you feel no different.  Ask your doctor.  In fact, drill your doctor with every question you can think of.   Doing this will help you understand the treatment process and how it will affect you.
Set up a Google search, subscribe to an arthritis publication or consider checking this Rocky Mountain Arthritis site often for news and new treatment methods.  Educating yourself (remember to ask questions) and not just relying on your physician is the key to getting the best treatment available.
